We are going to ask you a few questions to help direct your complaint to the right place.
Please include how you know this incident occurred; a description of the incident, names, and contact information of other witnesses or victims; and any other pertinent information.
If we do not have your name and contact information, we will not be able to contact you about this matter. Investigations are generally more effective if you are available to provide information and documents on request. If you choose to be anonymous, you may limit our ability to conduct a complete investigation, if warranted, of your matter.
We will not disclose the identity of an employee who comes forward with a complaint or information, unless the inspector general determines that such disclosure is unavoidable during the course of an investigation. Additionally, the Inspector General Act protects employees from reprisals or retaliation for having reported a complaint or disclosing information to us, unless there is willful or knowing disregard for the truth.
